To Register as a Parishioner

Fill out a  Parishioner Registration form and return to the Rectory office directly or by mail to 76 Broad St., Bloomfield, NJ  07003.

Please be sure to fill out all information: first and last names of all family members, all contact information including email addresses, birth dates, and all sacraments received along with dates (if possible).  Please print clearly.
